Form 956 attached. Please complete question 13 on page 4 and sign at question 24 on page 6;
Form 80 attached. All adult applicants to complete one form each and sign at question 53 on page 18. Additional space is provided at question 54 on page 19, if required;
DHA eMedical Questions attached to be completed for each applicant;
